Learning Objectives
At this meeting, Dr. Donna M. Mancini presented on classifying, managing, and when to refer heart failure patients, and Dr. Beth Oliver provided an overview on heart failure rapid follow up and transition of care, and Dr. David W. Lam discussed medication management in Type 2 diabetes, newer medication options and their cardiac benefits.
- Outline optimal ambulatory management and medication considerations for diabetes and CHF
- Recognize the value of multidisciplinary teamwork to optimize disease management for diabetes and CHF
