Year in Review

As we step into 2020, we reflect upon 2019 — a year where we focused on building a more connected network and creating resources to help you better understand your patients. This included:

• Rollout of Lumeris Accountable Delivery System Platform advanced analytics to share actionable patient-level data in support of achieving population health goals. Use of this tool also decreased the burden of quality reporting for many practices.

Launch of regional pod meetings, which has created opportunities for you to talk through common issues you face in daily practice with your colleagues in the same geographic region. These meetings promote best-practice sharing, quality and efficiency improvements, and encourage open sharing and learning.

• Enhancements of resources and tools such as our website filled with practice resources, our monthly newsletters, podcasts, provider spotlights, how-to videos, and most notably the new provider search directory where you can download lists of providers by specialty, health plan, and county/zip code in PDF or Excel.

Increased support for providers in patient outreach efforts to close care gaps using both direct phone outreach and automated outreach tools such as text messaging and interactive voice response calls.

• Launch of our clinical pharmacy program, which has supported more than 400 annual wellness visits, and will continue to support our providers in offering patients disease management, medication review, and other support. 

• Hosting our first MSHP CME-accredited event, on effectively and economically using medical assistants and clinical pharmacists in primary care practice.

• Publicly and formally recognizing our outstanding providers through our Provider Spotlight Series and CI Index Awards Ceremony 

• Distributed over $4.5 million in incentive payments and care coordination payments

• Demonstrating our commitment to information exchange by requiring all CIN providers install or have under contract a MIPS-certified EMR

• Conducted our 2nd annual Provider Experience Survey, where, as a CIN, you reported you were more likely than in the previous year to recommend a friend or colleague join the CIN, that you improved your quality of care, had less administrative burden, accessed otherwise-closed payer networks, and improved the economics of your practice.

• Welcoming 4 newly-elected physicians to the Board of Managers. We sincerely thank all who participated in the election for your commitment to physician-led governance.

• Piloted the launch of admission and discharge summaries with a small cohort of your peers; rollout to the larger network coming in 2020
